Payment
We require full payment when services are rendered. We accept cash, personal checks, money orders and for your convenience Visa, MasterCard, Discover and CareCredit.
Dental Insurance
If you have dental insurance, we are happy to process it for you if you supply us with all the necessary information. You can choose to assign your insurance benefits to Dr. Bork provided you complete and sign the assignment and release section of our form. We will then forward the claim to your insurance company for possible payment. We cannot make these arrangements unless we have all the information prior to your appointment . If we do not have the information, we require full payment at time of service.
We participate with most major dental insurance plans such as Aetna PPO, Aetna Dental Access, Delta Dental Advantage, Delta Dental Primere, Metlife PDP, Horizon Blue Cross - Blue Shield of NJ Traditional plan, Cigna PPO, Guardian Preferred and many more. We also particiapate with a few Union Local dental plans such as the Local 825. Please inform us of your plan prior to your appointment so that we may confirm coverage and our level participation.
CareCredit
Since we do not offer financing through our office we do work with a company called CareCredit. CareCredit is a company that helps people finance their dental and medical expenses. You may be eligible for a low or NO interest payment plan.
